The one that comes to mind is:

John Lucas, USA, basketball. Played in the NBA and was considered a standout point guard until his habits destroyed his career. Later in life, he underwent successful rehab and managed to become an NBA head coach.

Theoren Fleury, Canada, hockey. Alcoholism became well-documented later in his career; many traced it to his trying to cope with the sexual abuse he suffered under one of his junior coaches.

I believe there was also an American basketball player named Vin Baker who suffered from alcoholism, but am less sure on that one.
